Here is another attempt to corroborate this. It usually looks less swastikay and this video is the exception, not the norm. However, it still isn’t good and clearly isn’t a line (which would be expected because “realism”) or a circle (because easy). I can’t think of a way to do bullet spread that accidentally results in a swastika.

the question is, why would you make the spread like that and not just a circle? maybe to make the shotgun a little more effective as it would prefer to hit the torso, limbs, and head at close range? only reason outside the obvious that i can think of. though someone digging through the code/files could confirm if the spread is actually based on a swastika
That makes sense. My money is on it being a cross, (because the optimal spread for damage is a very tall cross) with inaccuracy being a product of distance from origin to prevent the frustration of missing every pellet of your shot. This would result in the average distribution of pellets looking like a cross potent, which has a chance of looking like a swastika. I don’t think this was intentional now that I think about it more.
