Patch #3 is now live for Baldur's Gate 3.This update brings the full release of Baldur’s Gate 3 to Mac, and allows you to change your character’s appearance in-game via the Magic Mirror.
I have no idea what happened but my performance and FPS increased by 10-30 FPS.
For comparison I normally play at ~20 FPS and now in some locations I have stable 50. That’s crazy. I actually increased my details to high and it’s still playable. Thank you Larian.

Performance improvements was one of the main points of this patch.
