Mine is Kingdom hearts when I was 10. I was halfway through the game when I jumped on the next morning, and it was just gone.
Now I’m overly cautious and have trust issues.

If any game requires only one save file, which it chooses when it automatically saves over it, I immediately have a distrust of it. All it takes is one thing to corrupt and you lose everything.
Mostly game on PC so storage isn’t an issue. I should be able to save when I went, to a new slot when I want. If that means I end up with 100 saves and it eats up a few hundred MB of space, so be it.
One of the ps1 final fantasy games either 7 or 8 lost 2hours because I didn’t check
Panicking after any hard section in a game usually so I do it without even realizing.
The Sexy Brutale is a good game, but it’s only autosave and you can’t access the savefile (it isn’t created in the expected folder). In addition, there’s this nasty bug that deletes your progress, it isn’t super-common but can’t be prevented (it happened to me).
Final Fantasy 6 on SNES. It didnt matter if I double saved or not though. I think it had a faulty battery and would lose its saves randomly all the time.
I remember one time, I was literally at the final boss fight and I saved and quit to go have dinner and when I came back, all the saves were gone.
I usually double save every game. I don’t trust myself and my non-stop brain.

Before Reddit I didn’t realize it was such a thing. I never saw friends do it or anything.
Pokemon soulsilver when I was younger. My ds would sometimes freeze or glitch, so it was very important to have numerous saves very often unless i wanted to lose a bunch of progress.
Played through to right before the final boss of System Shock 2, making sure to do just about everything on the way, just using quicksaves.
Right before dropping down to the final boss, I noticed I could attempt to parkour up some geometry over the abyss.
During one fall, I accidentally pressed quicksave instead of quickload.
There was no autosave.
